Hopedale Youth Baseball

This page contains the links to the registration forms for all the leagues sponsored by the HYBA. If your child attends the Hopedale school system and is within the age categories, they are eligible to participate in the program, even if you do not reside in the town of Hopedale. There are 2 types of registration forms for the Spring season. Your selection is based upon the which league your child will be participating in. The registration fees are listed on the forms below. You can mail in your registration form, simply right click on the displayed form, print and mail to the address list at the top ( with registration fee; check only please).

Medical Consent

Every player must complete the medical consent and information form in order for their child to be registered within the HYBA program. Please note any special considerations for any medical conditions.
